Many parents wonder if clear aligners can be used to treat children in mixed dentition cases. Many dentists are wondering the same thing 😊. The answer is yes, children can wear them with excellent compliance rates as they have worn other appliances in the past. Preventive and interceptive orthodontic treatments are invaluable in helping young patients achieve healthy and beautiful smiles in their future.

Preventive orthodontics involves monitoring tooth development and jaw growth to ensure everything stays in balance, while interceptive orthodontics focuses on correcting problems before they become more serious. Early intervention helps prevent more serious issues from occurring, ensures healthy mouths into adulthood and beyond, and could even save time and money in overall treatment time.

When treating young patients with orthodontic treatment, it is important to make sure the parents understand the possibilities and options available to them. Scientific literature suggests that clear aligners and traditional orthodontic appliances display similar efficacy and efficiency for maxillary incisor position corrections in the mixed dentition. The best appliance for any treatment is the one the patient will wear.

By understanding the principles of clear aligner orthodontics combined with growth and development principles, occlusion development, the appropriate age for treatment, and monitoring mixed dentition patients, clear aligners can be used to treat children in mixed dentition cases.

 Last Month's Answer and This Month's Question 

LAST MONTH, we posed a question about whether it is better to use a horizontal or vertical attachment for achieving root tipping (mesio-distal movement) on a tooth.

We received various responses, but Dr. Milton Ntragatakis provided an excellent answer: "…all the way!" And indeed, he is absolutely correct! This type of attachment creates a couple, which consists of two equal and opposite forces separated by a perpendicular distance. 

In a classic article published in 1995 in Seminars in Orthodontics by Isaacson (The ground rules of archwire design, Vol 1, No 1), it was explained that "the location of a couple on a tooth is irrelevant to the resulting tooth movement." Essentially, the position where the couple is applied on a tooth does not affect the outcome of the movement.

Understanding this concept may not be easy (trust me, I know!), but it is crucial to grasp. Take the time to ponder it and let it sink in.

Now, let's move on to THIS MONTH'S QUESTION, which pertains to mechanics—a topic I personally find fascinating. When a force is applied to a tooth, there are four possible outcomes. Here are the first three:


1. Rotation

2. Translation

3. Combination of rotation and translation

April was a busy month for The CLEAR Institute. Dr. Stephane Reinhardt was lecturing in South Carolina to the American Board of Pediatric Dentistry College of Diplomates. And they surprised him with something unexpected... When they were confirming by email if he needed anything special for his room or the presentation, he answered as a joke that he wanted a big bowl of  M&M's… But only red ones. Also, they told him that there would be a little get together after the lecture on Saturday and he said: "I hope there will be a giant tooth pinata contest".

Well guess what?!!!! A big bowl of red m&m's were waiting for him in his hotel room. And after the lecture on Saturday, guess what was waiting outside hanging on a tree?
